How to Register as a patient

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. You will still need to download and complete the following form and bring it in to Practice when you can.

A GP surgery can refuse to register a patient if the:

patient lives outside of the surgery catchment area
patient has been removed from the surgery they have tried to register with
patient lives a long way from the surgery and needs frequent visitation

The Register with a GP surgery service automatically checks your current address and determines if it is outside of our specified inner boundary area. If you are outside of this catchment area, the service will inform you and give the choice to continue or select an alternative GP surgery. Patients that choose to continue are warned that the registration could be declined by the GP surgery. The registration email sent to practices will highlight out of catchment registrations. Please be aware your registration request WILL be rejected if you are outside our boundary.

If you can’t register online

Please come to our reception desk. We ask ALL prospective new patients to bring one form of identification  and proof of address at registration. This is so we know you are who you say and your address is within our boundary. Children registering at the same time as their parents are exempt. We are happy to accept many forms of ID – see appendix 1 on our registration booklet .  We check the Visa Status of ALL foreign Nationals  to ensure patients are entitled to Secondary Care treatment on the NHS as we inform hospitals of entitlement at the point of referral.

New Patient Health Check

All new patients are invited to attend the Surgery to review their past medical history and medication. The receptionist will arrange this appointment with the practice nurse for you and you will be asked to complete a medical history questionnaire . Patients have the right to express a preference to receive services from a particular practitioner generally or in relation to any particular condition.
