If your clinician has requested a blood test from you this can be booked at the Practice by contacting our reception desk.
You may prefer to have your blood test taken at any of the following Community Phlebotomy services as as these options may be quicker or more convenient than in the Practice.

- Fleetwood Well Being Centre, 2nd Floor
The Whitegate and Fleetwood services offer appointments 09:30 until 16:00 Monday to Friday. Please follow the instructions below to book your test directly with any of these services or telephone 01253 956719
You will need to know your NHS number and create an account with the booking service but once this is done it’s straight forward.

IMPORTANT Patient information
Please note that bloods cannot be taken without a fully completed request card (CyberLab form). This will be provided by your clinician and is necessary so that the phlebotomist knows the correct type and number of sample tubes to be taken.
